To schedule an appointment with Dr. Porter, please call 701-235-0161 or use our online Request an Appointment form. If at any time you are unable to make your appointment, please notify us at least 24 - 48 business hours in advance. We would be glad to reschedule your appointment at a more favorable time, if necessary. We greatly appreciate your time and consideration and look forward to seeing you.

Once your appointment has been confirmed you can download and print the New Patient Forms. PLease fill out these forms and bring to your first office visit, this will save you time getting in to see the doctor.
